To attend the conference, you will need to complete Application Form VAF1C - Business Visitor.  To download this form, please click here Physiology 2011.

It is recommended that you apply for your visa well in advance (at least 6 weeks before the meeting).

The Society is happy to provide you with documentation to support your application.  However you must register to attend Physiology 2011 before we can provide this.

  • Date of birth
  • Address
  • Telephone number
  • Passport number
  • The purpose of the trip (i.e., to attend Physiology 2011).
  • How you intend to stay in the UK. 
  • Details of your accommodation
  • Details of how your living expenses will be covered
  • Date you intend to leave the UK
